Mountain-Pacific is devoted to improving our State’s health. One way to achieve this is to target the diseases where we as a community can make a difference. Our goal is to empower the people of our state and support their healthier choices. We will help our community by providing access to easy to understand information and resources that will provide knowledge and opportunities that support the unique needs of our growing older adult population.

Heart disease is the leading cause of death for men and women over the age of 65. Mountain-Pacific Quality Health works with primary care providers to identify and spread best preventive practices that enable patients to control their blood pressure, manage their lipids, learn about aspirin therapy and quit smoking.

Up to 80 percent of all strokes can be prevented by working with a healthcare professional to manage risk. Although stroke can happen to anyone, certain risk factors such as high blood pressure and diabetes can increase chances of a stroke. By following the latest prevention guidelines and filling out a stroke risk scorecard to discuss with a doctor, you are on the path to preventing a future stroke.
