We empower patients to participate in health care decision making and take an active role in managing their health.
Learning all you can about conditions you have is vital to good health. Equally important is choosing the right health care provider. The Centers for Medicare & Medicaid Services (CMS) provides information on the quality of care given at all Medicare-certified nursing homes nationwide. Together with the Alliance for Health Quality, CMS also publishes information about hospital care, providing data from reporting hospitals in the areas of surgical infection prevention, heart attack, heart failure, pneumonia and more.
You can access this information about your local providers by going to Medicare's compare sites, Nursing Home Compare and Hospital Compare. These resources constitute a good first step in selecting care for yourself or your family. Discussions with your physician, other health care providers, and family and friends should also provide you with useful information.
NOTE: Mountain-Pacific is committed to providing access to its Website for individuals with disabilities. We do this primarily by complying with Section 508 of the Rehabilitation Act requirements. Section 508 requires that individuals with disabilities, including Federal employees, have access to and use of information and data that is comparable to those without disabilities. Mountain-Pacific strives to meet this goal except when comparable access would result in undue burden.
If you use assistive technology (such as a Braille reader, screen reader, TTY, etc.) and cannot access information due to the format of the material, please contact Mountain-Pacific by phone at 1-800-497-8232.
